In an effort to minimize the delays and hassles of “filling out forms” at your first visit, we have implemented an on-line registration process. During this process, your membership account, medical chart and patient portal account will be established. Now, when you arrive for your first visit there will be no "paperwork or forms to fill out"!

Please read the below information carefully before starting the online registration process.

STEP 1: Pre-Registration
Please set aside approximately 15 minutes to complete the registration process.
A credit card is required for registration. We accept VISA, Master Card, American Express, Discover, JCB, and Diners Club.
Please have all your information at hand, including health insurance and credit card, as this will help speed up the registration process.
Please carefully read the Member Agreement before beginning the registration process.
STEP 2: Registration
Click on the "Register as a Member" button below and complete the online registration form.
Once you click the "Sign Up" button, your Registration Fee of $125 and first month's Membership Fee of $125 are charged to the credit card provided and your medical chart is established.
We will receive a message that you have completed registration.
STEP 3: Post-Registration
Next, a member of our staff will contact you to help set-up your Patient Portal account, which has many features to enhance your care at Infinium Med.
Our staff will send the necessary "New Patient" forms to your Patient Portal. Please log in to your Patient Portal and complete the these documents.
After you complete all the documents, please notify us so we can schedule your initial appointment. You can scan the completed documents and attach them to a message to us in your Patient Portal (preferred) or bring them with you on your initial appointment.
