In an effort to minimize the delays and hassles of “filling out forms” at your first visit, we have implemented an electronic enrollment ("e-enrollment") process. During this process, your membership account, medical chart and patient portal account will be established. Now, when you arrive for your first visit there will be no "paperwork or forms to fill out"!

Please read the below information carefully before starting the e-enrollment process.

STEP 1: pre-enrollment
Please set aside approximately 15 – 30 minutes to complete the registration process.
A credit card is required for registration. We accept VISA, Master Card, American Express, Discover, JCB, and Diners Club.
Please have all your information at hand, including health insurance, credit card, and medical history information, as this will help speed up the enrollment process.
Please carefully read the Member Agreement before beginning the enrollment process.
STEP 2: ENROLLMENT
Click on the "Enroll" button below and complete the e-enrollment form.
Once you click the "Sign Up" button, your enrollment fee and first month's membership fee are charged to the credit card provided and your medical chart is established.
We will receive a message that you have completed enrollment.
STEP 3: Post-Enrollment
Next, a member of our staff will contact you to help set-up your Patient Portal account, which has many features to enhance your care at Infinium Med.
Log in to your Patient Portal, click on the the "Questionnaires" tab and complete the documents in the "New Patient" section.
After you complete all the documents, we are notified by our EMR system that they are ready for review by our staff and your Personal Physician.
